At Jewell + Kudro, we believe good care begins with understanding what makes a person who they are. A diagnosis and medical history tell us part of the story, but so does the life that came before: what matters to them, what they miss, whom they love, and what still brings them joy.
Our care joins skilled nursing with meaningful human connection. That may mean managing medications and chronic illness, accompanying someone to a physician appointment, sharing coffee over old photographs, returning to a favorite bookstore, or helping a family navigate the final chapter of a life.
Whether someone is recovering from illness, living with dementia, becoming more isolated, or approaching the end of life, we begin by listening. We learn their stories, routines, relationships, and the small things that make a life feel like their own.
Because the goal isn’t simply to keep someone safe, wherever they call home. It’s to help them remain themselves there.
Skilled Nursing Care

For older adults who need clinical support while remaining at home, Skilled Nursing Careprovides consistent, relationship-based care from a nurse who comes to know the person, their medical history, and their changing needs.
Care may include medication management, ongoing clinical assessment, chronic disease monitoring, wound and skin care, post-hospitalization support, communication with physicians and specialists, accompaniment to medical appointments, and education and guidance for patients and families.
Between visits, we remain engaged in care, coordinating with families and medical providers and responding as needs evolve.

Narrative Nursing & Community Life
Connection. Meaning. A life still being lived.
For older adults whose worlds have grown smaller through illness, isolation, loss, or changing circumstances, Narrative Nursing & Community Life combines nursing oversight with meaningful companionship and intentional engagement.
Care is shaped around the person’s history, interests, relationships, and abilities. A visit might mean returning to a favorite bookstore, sharing lunch at a familiar café, visiting a museum or library, taking a walk, looking through old photographs, working on a memory or legacy project, or rediscovering an interest that once brought joy.
The goal is not simply to fill time. It is to create opportunities for connection, purpose, curiosity, and participation in the world.

Private Care Management
Comprehensive oversight. Thoughtful and trusted coordination.
For families navigating complex or increasing care needs, Private Care Management provides experienced oversight of the entire care picture. We serve as the central point of contact among the patient, family, caregivers, physicians, and other providers, ensuring that care remains coordinated as needs change.
We assess needs, develop individualized care plans, coordinate medical providers and medications, recruit and oversee private caregivers, organize schedules and routines, monitor changes in health and function, and maintain regular communication with the family. When extended or around-the-clock support is needed, we can build and oversee the larger care team.
The goal is to give families confidence that someone knowledgeable is paying attention to the whole picture, so they can spend less time managing care and more time simply being family.
